CDL/CDL Learner Permit: * (9 digits)	Confirmation By choosing NI (Non-excepted Interstate), you are required to make an appointment with a DMV branch to have the “K” restriction removed from your license and pay a fee for a duplicate license. Confirmation By choosing non-excepted intrastate (NA), a “K” restriction will be added to your license which will restrict you to operating a Commercial Motor Vehicle (CMV) within Connecticut only. You must make an appointment with a DMV branch to obtain a duplicate license with the “K” restriction, and pay a fee for the duplicate license.

To satisfy the requirements of 49 CFR 383.71, you must check the self-certification category that applies to you. Please check only one category. You must also provide a medical certificate. Please upload a copy of your current medical card to https://cdlmedcert.ct.gov I certify that I drive or expect to drive a commercial vehicle in:
Category 1 – (NI) – Non-excepted Interstate I will operate a commercial motor vehicle both in and outside the boundaries of Connecticut. Select this category only if you are 21 years of age or older and do not have or require a "K" or "W" restriction on your Commercial Driver's License.
Category 2 – (NA) – Non-excepted Intrastate I will operate a commercial motor vehicle only within the boundaries of Connecticut. Select this category if you are: Under 21 years of age or Maintain or require a medical waiver "W" restriction on your Commercial Driver's License, or Choose to add or maintain an intrastate-only "K" restriction on your Commercial Driver's License.
You must obtain a duplicate of your current CLP or CDL reflecting any changes based on your self-certification category selection.
